SPECIMEN LABELING

SPECIMENS MUST BE LABELED WITH 2 UNIQUE PATIENT IDENTIFIERS

EVERY individual specimen sent to the lab must have a label applied to the tube, container, bag (not loose in the bag), vial, etc.

Unique Identifiers: Patient name, DOB, MRN, etc.

1. If the label already has a unique identifier (such as a serial number), only 1 additional identifier is required (full name).

2. Multiple Specimens: Must include anatomic location for each. Label must include designation corresponding to anatomical site (A, B, C or D).

Fixative: Verify the expiration date before using fixative filled container.

Zamboni Fixative: ENFD Only
Copan eSwab buffer: Any PCR Sample
Formalin: Histology testing
Sterile OR ETOH: Aspirate
Dry: Nails
Packaging:

A. Specimen containers are placed directly into the bio-hazard bag.

B. Requisition form and demographic sheet should be folded and placed into the outside pouch of the bio-hazard bag.

C. Multiple bio-hazard bags can be shipped in the UPS Laboratory Pak together. **Please send all nail samples using the white “Nail only” mailers.
